Patient Access - Online Services

Patient Access has been in use in the surgery for some time now and many of our patients are happily using it to book appointments. We have now extended the facilities offered using this system to include ordering your repeat prescriptions, updating your contact details, sending secure messages and viewing a summary of your medical record.  From 8am each morning you will also have access to all of the available appointments for that day - avoiding the need to phone during our busiest time. 

 

HOW DO I REGISTER?

 

If you would like to register for yourself you will need to complete and return the Patient Access application form available by clicking the link below or from reception.

If you are applying for access on behalf of another patient you will need to complete the following application form.  

 

Patient Access - Parent-Carer Application Form

Please complete the form in full but do NOT sign it. When you return this to reception you will need to provide 2 forms of identification (one to include your photograph where possible) and sign it in front of reception staff. Your application form will then be passed to our admin department who will email you your registration letter detailing all of the information required to create your online account. (When you register online please remember to note down your user ID and password as this will be needed each time you access the service). If you are applying for access for another patient you will also need to provide proof of your right to access their information (details of the types of documentation you can provide can be found on the application form).
