All new patients will be expected to attend for a health check prior to being fully registered. To register ask for a form from reception & make an appointment to see one of the practice nurses. When you attend bring the form - NHS card and any medication you may be taking.
If we refuse an application to register we will notify you within 14 days with a reason for refusal. A record of refusal will be kept and can be made available to the CCG.

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.
