CHANGE TO HOW YOU BOOK APPOINTMENTS 2025.

All requests for a GP appointment will now need to be submitted by completing a medical request form available on our surgery website Contact us about your request – Accurx Patient Portal or via the NHS app.

You can submit medical request forms from 6:00am, Monday to Friday so please complete your form early to help us offer you the earliest appointment, where appropriate. Between 2pm and 6.30pm (Mon-Fri), if you feel your condition requires same-day care, you now need to RING the surgery on 01257 442665. If you have a routine care or administrative query, this will be responded to within 1 working day.

If you do not have access to the internet, or find it difficult to use, feel free to give us a call and we will complete the medical request form with you over the telephone.

What information is required?

When filling out the medical request form, you will need the following details:

  • Your full name
  • Date of birth
  • Contact details (phone number and/or email)
  • Brief description of your medical issue or request
  • How long you’ve had the symptoms
  • Any medications you’re currently taking
  • Any other relevant medical history

This information helps our clinical team assess your needs and direct you to the most appropriate care.  Please include as much detail as possible when completing the medical request form.

What happens next?

Once you have completed the medical request form, a GP will review it. We will then contact you either by phone or text.

  • If we call, it will be to book an appointment with the most suitable person.
  • If we text, you will receive a link to book with a specialist clinician.

The GP will also determine whether your appointment should be by telephone or in person.

Forms will then be triaged and appointments offered depending on urgency and availability.

What if I don’t have access to the internet?

If you do not have internet access or need help completing the medical request form, please call our reception team. A team member will fill it out with you, and it will be processed just like any form submitted online.

Can I still book an appointment in person?

From 29 September 2025, all appointment requests will need to be submitted via a medical request form so they can be appropriately assessed by a clinician.

What if I need a little extra help?

If you need help accessing the medical request form online or through the NHS app, please speak to reception or pop into the surgery — a member of staff will be happy to assist you.

How Can You Prepare for This Change?

If you haven’t already, we encourage you to download the NHS App on your mobile device via the Google play or App store. You can also access the same services in a web browser by logging on through the NHS website NHS App and your NHS account – NHS

Please make sure we have your current mobile number so we can contact you quickly with updates, appointment information, or important messages.

If you are unable to complete the medical request form, our reception team is happy to help. We can assist patients who don’t have internet access or need extra support by completing the medical request form on their behalf — so everyone can access the care they need.
