Sick (fit) notes

If you’ve been off sick for more than 7 days then you may need to show your employer a fit note (also known as an NHS sick note) from your GP to confirm that you’ve been ill and that you’re able to return to work. You can request a GP fit note using our online system. Find out more below.

How to get a fit note

You can request a fit note using the link below, to complete a form. Once you have completed the form, your GP will get in contact to either issue a fit note to say that you’re fit to work or book an appointment to see you face-to-face. This depends on your illness and whether you’ve been in hospital.

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
