To register with our surgery is a straightforward process using the link below. Complete the quick online form to start the process. No need for proof of address, immigration status, ID or NHS No.

The online process is quick and easy and checks that your address is within our practice boundary and also matches you with your NHS No. The registration form must be completed and processed before you can use our services. When you register you will also be asked to complete a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

Upon registering, you will be allocated a named accountable GP.  You will be notified of this shortly after your registration with the practice has been processed.  Your named GP will have overall responsibility for your care but this does not prevent you from seeing any available GP or Nurse in the practice.

If we refuse an application to register we will notify you within 14 days with a reason for refusal. A record of refusal will be kept and can be made available to the ICB.
