New Patient Registration

Catchment Area

All new patients will be expected to attend for a health check prior to being fully registered. To register, please ask for a form from reception & make an appointment to see one of the practice nurses. When you attend bring the form and any medication you may be taking.

Upon registering, you will be allocated a named accountable GP.  You will be notified of this shortly after your registration with the practice has been processed.  Your named GP will have overall responsibility for your care but this does not prevent you from seeing any available GP or Nurse in the practice.

If we refuse an application to register we will notify you within 14 days with a reason for refusal. A record of refusal will be kept and can be made available to the CCG.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
