Redesigning Our Recall Process for Patients with Long-Term Conditions

Here at Granville House Medical Centre, we are committed to continuously improving our services to better meet the needs of our patients. With this in mind, we have recently made changes to our recall process for patients with long-term conditionsUnder our redesigned approach, we will now be inviting patients with long-term conditions to attend appointments or submit Blood pressure readings based on their month of birth. This adjustment enables us to streamline our scheduling process while ensuring that all our patients receive timely and appropriate care.

We understand that some patients may have had recent appointments with our nurses within the last three months. However, we wish to stress the importance of attending scheduled appointments, regardless of recent consultations. Regular check-ups and monitoring are essential for effectively managing long-term conditions and promptly addressing any changes in your health.

As a further note, we would like to inform you that diabetic patients born in January will be called in sooner, typically in November or December. This adjustment is to accommodate any indulgence during the Christmas period and ensure that your health needs are adequately addressed.
